Armstrong Cooper Youth Hockey

It is the responsiblity of the Head Coach to ensure players are appropriately supervised in the locker rooms.

Coaches and team managers, ensure you and your supervising adults are familiar with the attached guidelines, i.e. no cell phones in locker rooms, etc.

Parents/adults, unless disqualified, it is your responsibility to assist with locker room supervision for your team.  

1. Complete form on-line using link below.

• You may be required to create a log-in with MN Hockey if you have not already.
• We are in District 3 and use the full association name - Armstrong Cooper Youth Hockey Association
• If you are not a coach (or board member), indicate you are a Manager when choosing a position

2. Do a screen print or print the summary receipt upon completion.
3. Submit form electronically.
4. Submit paper summary receipt to Team Manager or Registrar.

IMPORTANT:  If you have already completed ONLY a paper background screen form for the 2011-12 season, you ARE required to complete the on-line form and submit a copy to the Registrar.
