2011-2012 Registration



Online registration opens on August 15, 2011 and ends September 5, 2011. For returning hockey players this is the only way you can sign up for hockey for the 2011 - 2012 season.

Peewees, Bantams, U12 and U14: Start September 12, 2011

Squirts/U10's: Start near end of September , 2011

Mites/U8's: Start October 1, 2011


If you have a past due bill from a previous season, you must settle it before registering this season. If you do not settle it, your current registration funds will be used to settle it and your player will not be allowed to skate until you’ve paid up for the 2010-2011 season. Step 1: Sign up and register with USA Hockey

 

A.  Go to the USA Hockey Website and Register CLICK HERE 
B.  Choose member type Ice Player/Coach.
C.  You must be 18 years of age to process a registration.
D.  Check the box that acknowledges that YOU are 18 or older.
E.  Select the correct season – 2011-12.
F.  Follow the prompts...fill in all data correctly.
G.  Process payment – Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work.
 

You may print your Confirmation page at this time. It will also be emailed to you immediately after the registration is completed.

Each participant will pay the USA Hockey fee of $40 if age 7 or older. The USA Hockey fee is waived for age 6 (birth year 2005 or later) and younger.  There is a $30 USA Hockey fee and a $10 affiliation fee. The registration must still be completed to get the confirmation number.

Once you have registered all players with USA Hockey, and have your confirmation pages, please return to this site to register with ACYHA.

Each player must have their own unique USA Hockey confirmation number.


Step 2: Sign up for an Account with ACYHA (If you are a returning family, its likely you already have one)

In order to register as a hockey player in the ACYHA. you must have an account with the ACYHA Website.


Step 3: Register with ACYHA

ALL RETURNING PLAYERS MUST REGISTER ONLINE:  A $75.00 late fee will be assessed after September 4.

Make sure you have your USA Hockey Confirmation Code- this is REQUIRED to register with the ACYHA.

NEW MITE INFORMATION NIGHT:
We will hold an information night on Sept 20 from 6-8pm in Meeting Room 1 for all new mite families.  You may register online or bring your checkbook and registration forms to the arena.

Step 4: Complete paperwork - do not mail!

The following forms must be submitted in conjunction with your on-line registration.  No player will be allowed to be on the ice without these forms properly submitted. 

Required paperwork and deposits:

Copy of birth certificate - If not previously submitted

USA Hockey Consent to Treat/Medical History Form

USA Hockey Code of Conduct - Traveling players only

$500 Volunteer deposit check - payable to ACYHA

$50 Fundraiser deposit check - payable to ACYHA


Step 5: Bring paperwork/deposit checks to registration drop off night

All families must turn in both their Volunteer and Fundraiser Deposit Checks if you DID NOT opt out. Make checks payable to ACYHA.  This is a requirement for your child to participate.

Please write your child’s name in the memo line of your checks.

Your checks will be returned following the completion of your volunteer hours and fundraiser. If you do not complete ALL of your hours or participate in the fundraiser, your check will be cashed.
