Coaches Screening and Backgrond Checks

Coaches Screening Form

All Coaches, Board Members, Managers, and Locker Room Supervisors must fill out an on-line Background Screening Form through MN Hockey before they can go on the ice this year or participate in association activities. Please complete the on-line submission AND print out the summary receipt.  Place the summary receipt in the Youth Hockey Office in the Registrar's mailbox.

To coach at any level for the ACYHA you must complete an online Background Screening form: (use your log on from the ACYHA website)

  1. www.minnesotahockey.org
  2. Log in, or create log in
  3. Follow through all of the forms and submit.
  4. Give hard copy of summary receipt to association registrar

We are in District 3 and use the full association name - Armstrong Cooper Youth Hockey Association.
