Team Emergency Contact Information

At the direction of the ACYHA board, as team manager(s), we ask that you solicit emergency contact information from the parents/guardians, complete the Team Emergency Contact Information template attached below, and distribute laminated or protected copies as follows:

·         1 copy in team puck bag
·         1 copy in team first aid kit
·         1 copy in manager book
·         1 copy to each coach
·         1 copy emailed to Cristy Siede for placement in the Youth Hockey Office binder

It is critical you solicit this contact information directly from the parents/guardians at this time for this purpose; do not rely on registration information.  You may pre-populate the form with info you have and ask them to confirm.

The purpose of this exercise is to have this contact information in several locations that are quickly accessible to the coaching staff in the event of an injury/emergency.

Deadline:  Information should be distributed no later than Monday, December 26, 2011.

If you have any questions, please don’t hesitate to contact any of the following:

Greg Symicek, ACE and Coaches Coordinator
Brad Caron, Boys Travel Coordinator
Anne Winter, Girls Travel Coordinator
Kevin Conway, Youth Director
Dave Parenteau, Assistant Youth Director
Cristy Siede, Secretary

Thank you for your help.
