COVID-19 Return To Hockey

The Armstrong Cooper Youth Association would like to provide our hockey community with a landing page for resources as we begin to Return To The Rinks and Return To Hockey. We will continue to update this page as new information and resources become available.

THE QR SCREENER BELOW MUST BE DONE BEFORE EVERY PRACTICE AND GAME (HOME OR AWAY) - Please complete one for each player, coach and parent spectators.  If your player has a spectator other than a parent, please complete one for them, too.  Mite/U8 parents - if you will be staying at the rink, please complete one for yourself, too.

Notification to ACYHA Of COVID POSITIVE TEST

ACYHA Covid-19 Test Result Notification

Please fill out this questionnaire to notify the ACYHA COVID Task Force of a COVID -19 test result. This questionnaire is for reporting results for the 5-7 day quarantine window or any COVID-19 positive test results.

ACYHA Covid-19 Exposure Notification

Please fill out this questionnaire to notify the ACYHA COVID Task Force of a COVID positive exposure. The information in this Questionnaire will assist the Task Force to determine 10 day or 14 day quarantine and if criteria is met for 7 day quarantine.

MN HOCKEY UPDATES

Please note the following from the MN Hockey Phase 5 Return to play guidelines: "Each association should have its own plan in place for returning to the rink and be prepared for the possibility of positive cases occurring within participants or their families. Local rinks and associations need to follow the guidelines set forth by the Center for Disease Control and Prevention (CDC), the Minnesota Department of Health (MDH) and local government or health officials in returning to the rinks and managing confirmed cases of COVID-19. The information in this document is not intended or implied to be a substitute for professional medical advice, diagnosis or treatment. These guidelines are designed to assist associations in creating their own emergency and alert plans for confirmed positive cases.
